To be eligible to apply for funding under this NOFA, you must be a state, Native American Tribe, city, county/parish, or other unit of local government. Multiple units of a local government (or multiple local governments) may apply as a consortium; however, a principal (lead) applicant must be identified that will be responsible for ensuring compliance with all requirements specified in this NOFA. State government and Native American tribal applicants must have an Environmental Protection Agency (EPA) authorized lead-based paint training and certification program.

The overarching purpose of the Lead Based Paint Hazard Control Program and the Lead Hazard Reduction Demonstration Grant Program is to assist states, Native American Tribes, cities, counties/parishes, or other units of local government in undertaking comprehensive programs to identify and control lead-based paint hazards in eligible privately owned rental or owner-occupied housing, with the exception that the Lead Hazard Reduction Demonstration Grant Program is targeted for urban jurisdictions with the greatest lead-based paint hazard control needs.

To be eligible as a library applicant for a National Leadership Grant, you must •be either a unit of State or local government or be a private nonprofit organization that has tax-exempt status under the Internal Revenue Code; •be located in one of the 50 States of the United States of America, the District of Columbia, the Commonwealth of Puerto Rico, the U.S. Virgin Islands, Guam, American Samoa, the Commonwealth of the Northern Mariana Islands, the Republic of the Marshall Islands, the Federated States of Micronesia, or the Republic of Palau; and •qualify as one of the following: a library or a parent organization, such as a school district, a municipality, a state agency, or an academic institution, that is responsible for the administration of a library. Eligible libraries include public libraries, public elementary and secondary school libraries, college and university libraries, research libraries and archives that are not an integral part of an institution of higher education and that make publicly available library services and materials that are suitable for scholarly research and not otherwise available. Research libraries, under the supervision of at least one permanent professional staff librarian, must be either generally recognized as possessing unique scholarly research materials and services that are made available to the public, or able to demonstrate that such is the case when submitting an application to IMLS. Private or special libraries that have been deemed eligible to participate in this program, by the State in which the library is located; an academic or administrative unit, such as a graduate school of library and information science that is part of an institution of higher education through which it would make application; a digital library, if it makes library materials publicly available and provides library services, including selection, organization, description, reference, and preservation, under the supervision of at least one permanent professional staff librarian; a library agency that is an official agency of a State or other unit of government and is charged by the law governing it with the extension and development of public library services within its jurisdiction; a library consortium that is a local, statewide, regional, interstate, or international cooperative association of library entities that provides for the systematic and effective coordination of the resources of eligible libraries, as defined above, and information centers that work to improve the services delivered to the clientele of these libraries; or a library association that exists on a permanent basis, serves libraries or library professionals on a national, regional, state, or local level, and engages in activities designed to advance the well-being of libraries and the library profession.

Description

National Leadership Grants support projects that address challenges faced by the museum, library, and/or archive fields and that have the potential to advance practice in those fields. Successful proposals will seek innovative responses to the challenge(s) identified in the proposals, and will have national impact. For FY2013, IMLS continues to pursue its commitment to early learning by seeking proposals addressing learners from ages 0-8 and their parents and caregivers. Library applicants are encouraged to partner with community organizations to address at least one of the challenges identified by the Campaign for Grade Level Reading including school readiness, summer reading loss, and chronic school absence. Projects addressing the goals of the Campaign for Grade Level Reading should check the appropriate box on the Program Information Sheet component of the application. The National Leadership Grant program accepts applications under three main categories: •Advancing Digital Resources—Support the creation, use, presentation, and preservation of significant digital resources as well as the development of tools to enhance access, use, and management of digital assets. •Research—Support research that investigates key questions that are important to museum, library, and archival practice. •Demonstration—Support projects that produce a replicable model or practice that is usable, adaptable, or scalable by other institutions for improving services and performance. Applicants may choose to submit a Project Grant, Planning Grant, or National Forum Grant proposal in any of the above categories. •Project Grants support fully developed projects for which needs assessments, partnership development, feasibility analyses, prototyping, and other planning activities have been completed. •Planning Grants allow project teams to perform preliminary planning activities that could lead to a subsequent full project, such as needs and feasibility analyses, solidifying partnerships, developing project work plans, or developing prototypes or proofs of concept. Applications for Planning Grants must include at least one formal partner in addition to the lead applicant. •National Forum Grants provide the opportunity to convene qualified groups of experts and key stakeholders to consider issues or challenges that are important to libraries, museums, and/or archives across the nation. Grant-supported meetings are expected to produce widely disseminated reports with expert recommendations for action or research that address a key challenge identified in the proposal. The expert recommendations resulting from these meetings are intended to guide future proposals to the National Leadership Grant program.

Description

This announcement solicits applications for the Small Health Care Provider Quality Improvement Grant Program.  The purpose of the Small Health Care Provider Quality Improvement Grant (Rural Quality) Program is to provide support to rural primary care providers for implementation of quality improvement activities.  Quality health care is the provision of appropriate services to individuals and populations that are consistent with current professional knowledge, in a technically competent manner, with good communication, shared decision-making and cultural sensitivity.  The ultimate goal of the program is to promote the development of an evidence-based culture and delivery of coordinated care in the primary care setting.  Additional objectives of the program include:  improved health outcomes for patients; enhanced chronic disease management; and better engagement of patients and their caregivers.   Organizations participating in the program are required to utilize an evidence-based quality improvement model, perform tests of change focused on improvement, and use health information technology (HIT) to collect and report data.  HIT may include an electronic patient registry (EPR) or an electronic health record (EHR), and is a critical component for improving quality and patient outcomes.  With HIT it is possible to generate timely and meaningful data, which helps providers track and plan care.  It is expected that applicants have implemented an HIT system by the time of award.  Implementation is defined by the Office of the National Coordination for Health Information Technology as installation of the system and associated activities, such as training, mock “go-live,” and pilot testing.  This program does not support funding for an EHR, but grantees may use funds to develop or purchase a module or interface or customize reports to support collection of data.  If any applicant plans to use an EHR for this program, it is highly recommended that the product is certified by the Office of the National Coordinator for Health Information Technology:  (http://healthit.hhs.gov/portal/server.pt/community/healthit_hhs_gov__standards_and_certification/1153). The Rural Quality Program supports three years of funding.  During the first year of the program, grantees will complete implementation of any additional HIT needed for data collection, train staff on data collection and the evidence-based quality improvement model, begin collecting, testing and validating data, and begin implementation of quality improvement initiatives and tests of change.  Grantees are expected to report baseline clinical data by six (6) months after the project period start date.  During year two, grantees will continue reporting clinical data, using data to guide quality improvement initiatives, and work on sustainability, culminating in submission of a final sustainability plan by the end of the year.  During the third and final year, grantees will continue reporting data, identify another condition or disease to begin quality improvement activities once federal funds have ended, and continue work on sustainability.  Although it is not a requirement, the Office of Rural Health Policy (ORHP) is strongly encouraging applicants to form a consortium or network for this program.  The health care system is becoming increasingly collaborative, and organizations will need to form partnerships and leverage resources to participate in many incentive and payment programs.  Through other grant programs, ORHP has found that consortia and networks can better meet community need, enhance each organization’s role, and stabilize and expand needed services and rural delivery systems.          Sustainability of project activities beyond the funding period is a priority of the ORHP.  It is expected that participation in this program will help prepare organizations to continue their quality improvement programs, participate in for pay-for-performance and other incentive programs, such as Patient-Centered Medical Home, Meaningful Use, and Accountable Care Organizations.  Grantees will also be required to submit a final sustainability plan at the end of the second year that includes an additional health indicator they plan to target for quality improvement and data collection once federal funds have ended.  The plan should explain why the health indicator was selected. Under health services delivery programs, HRSA funding may serve as seed money to allow recipients to develop necessary capabilities and the ability to obtain funding from non-Federal sources.  Recipients must maximize funding from other sources, using award funds for the difference between those amounts and their costs of operation.  Therefore, applicants must describe whether other funding sources and/or services currently exist for the proposed population and, if so, how HRSA funds would be used. To the extent possible, grantees are encouraged to bill for third party reimbursement for covered services in order to focus program dollars on the training of staff in quality improvement activities to build a culture of quality within their organization that will aid in the sustainability of the project beyond Federal funding.

The Arts Endowment’s support of a project may begin any time between January 1, 2014, and January 1, 2015, and extend for up to two years. The NEA Literature Fellowships program offers $25,000 grants in prose (fiction and creative nonfiction) and poetry to published creative writers that enable recipients to set aside time for writing, research, travel, and general career advancement. Applications are reviewed through an anonymous process in which the only criteria for review are artistic excellence and artistic merit. To review the applications, the NEA assembles a different advisory panel every year, each diverse with regard to geography, race and ethnicity, and artistic points of view. The NEA Literature Fellowships program operates on a two-year cycle with fellowships in prose and poetry available in alternating years. For FY 2014, which is covered by these guidelines, fellowships in prose (fiction and creative nonfiction) are available. Fellowships in poetry will be offered in FY 2015 and guidelines will be available in the fall of 2013. You may apply only once each year. Competition for fellowships is extremely rigorous. We typically receive more than 1,000 applications each year in this category and award fellowships to fewer than 5% of applicants. You should consider carefully whether your work will be competitive at the national level.

The Sparks! Ignition Grants for Libraries and Museums are a special funding opportunity within the IMLS National Leadership Grants program. These small grants encourage libraries, museums, and archives to test and evaluate specific innovations in the ways they operate and the services they provide. Sparks Grants support the deployment, testing, and evaluation of promising and groundbreaking new tools, products, services, or organizational practices. You may propose activities or approaches that involve risk, as long as the risk is balanced by significant potential for improvement in the ways libraries and museums serve their communities. Successful proposals will address problems, challenges, or needs of broad relevance to libraries, museums, and/or archives. A proposed project should test a specific, innovative response to the identified problem and present a plan to make the findings widely and openly accessible. To maximize the public benefit from federal investments in these grants, the Sparks Grants will fund only projects with the following characteristics: Broad Potential Impact—You should identify a specific problem or need that is relevant to many libraries, archives, and/or museums, and propose a testable and measurable solution. Proposals must demonstrate a thorough understanding of current issues and practices in the project’s focus area and discuss its potential impact within libraries, archives, and/or museums. Proposed innovations should be widely adoptable or adaptable. Significant Innovation—The proposed solution to the identified problem must offer strong potential for non-incremental, significant advancement in the operation of libraries, archives, and/or museums. You must explain how the proposed activity differs from current practices or takes advantage of an unexplored opportunity, and the potential benefit to be gained by this innovation.

The first funding cycle of FY 2013 is covered under EDA’s FY 2012 Economic Development Assistance programs (EDAP) FFO dated November 18, 2011. EDA strongly encourages electronic submissions of applications through www.grants.gov (Grants.gov). To be considered during a particular funding cycle, completed applications must be validated and time-stamped at Grants.gov by 11:59 p.m. Eastern Time on the applicable funding cycle deadline. Alternatively, paper applications may be delivered to the applicable regional office listed in section VIII. of this FFO. Paper applications must be received no later than 5:00 p.m. local time in the applicable regional office on the funding cycle deadline and the applicant must use a delivery confirmation service from their selected carrier. Current Closing Date for Applications:The deadlines for the remaining funding cycles of FY 2013 and the first funding cycle of FY 2014 are listed below. The first funding cycle of FY 2013 is covered under EDA’s FY 2012 Economic Development Assistance programs (EDAP) FFO dated November 18, 2011. EDA strongly encourages electronic submissions of applications through www.grants.gov (Grants.gov). To be considered during a particular funding cycle, completed applications must be validated and time-stamped at Grants.gov by 11:59 p.m. Eastern Time on the applicable funding cycle deadline. Alternatively, paper applications may be delivered to the applicable regional office listed in section VIII. of this FFO. Paper applications must be received no later than 5:00 p.m. local time in the applicable regional office on the funding cycle deadline and the applicant must use a delivery confirmation service from their selected carrier. If your application is received after the deadline, it will be considered late and will not be reviewed by EDA for that funding cycle. EDA will not accept facsimile or email transmissions of applications. Additional Information on Eligibility:

Pursuant to EDA’s authorizing statute, the Public Works and Economic Development Act of 1965 (PWEDA), eligible applicants for and eligible recipients of EDA investment assistance include a(n): (i) District Organization; (ii) Indian Tribe or a consortium of Indian Tribes; (iii) State, city, or other political subdivision of a State, including a special purpose unit of a State or local government engaged in economic or infrastructure development activities, or a consortium of political subdivisions; (iv) institution of higher education or a consortium of institutions of higher education; or (v) public or private non-profit organization or association acting in cooperation with officials of a political subdivision of a State. See section 3 of PWEDA (42 U.S.C. § 3122) and 13 C.F.R. § 300.3. EDA is not authorized to provide grants directly to individuals or to for-profit entities seeking to start or expand a private business.
