Description
- The HIRE Program seeks to improve the HIV/AIDS health outcomes of formerly incarcerated persons re-entering the mainstream population by supporting community-based efforts to ensure their successful transition from state or federal incarceration back to their communities. It is expected that the HIRE Program will: ? ensure that the re-entry population is sufficiently prepared to maximize their chances for successful reintegration; ? seek to eliminate the structural and medical barriers to reintegration that are necessary to preserve public safety; ? acknowledge the roles that families and communities play in the reentry process and ensure that the re-entry population is sufficiently prepared to take on those responsibilities; and ? collect the data needed from pre-release and post-release prison medical reports to better evaluate the effectiveness of the project and assist the re-entry population affected by HIV/AIDS. HIRE Program grantees, through a systems navigation approach, will provide access to prevention and treatment services to the re-entry population. Grantees are expected to form stakeholder partnerships that will plan, develop and provide community-based HIV/AIDS-related services, transitional assistance, and substance abuse and behavioral and mental health services for the re-entry population. Community-based and faith-based organizations will be provided the opportunity to deliver comprehensive HIV/AIDS-related services and transition assistance to include prescription drug assistance and substance abuse and behavioral and mental health services as well as issues surrounding employment, family, education, housing, and community involvement.
